Overnight Visitor Registration
For registering any off-campus visitor that is staying overnight, between the times of 12 a.m. (midnight) and 8 a.m. -
Criminal Disclosure Form
Printable form for students who must disclose criminal convictions. -
Damage Report Form
Residents should document pre-existing damages to their room/apartment at the start of the year. -
Facilities Request Form
Students looking to report any facilities-related problems should use this online form. -
Computing/IT Service Request
Students can submit a request to IT using this form. -
Housing Cancellation Form
Students may request to cancel their 2023-2024 housing application online by logging into Housing Portal and selecting Cancellation Form. -
Room Change Request Form
Assigned students can submit a room change request by logging into Housing Portal and selecting Room Change Request Form. -
Roommate Success Plan
Form for residents to complete together at the start of a new living arrangement. -

These are the terms and conditions that a student agrees to upon completing their housing application.
Many of these forms may be completed online. However, some forms cannot be submitted electronically. To return a paper form, you may either fax it to 724-836-7134 (ATTN: Housing Office), e-mail it to UPGHousing@pitt.edu, or send it via mail to:
